Plains View Surgery welcomes new patients who live within the practice area.
To register you will need to complete a GMS1 Family doctor Services Registration form and health questionnaire which can be obtained from reception. Please bring with you your medical card, proof of address, NHS number and details of your previous GP. Patients who are out of the practice area or are transferring from another practice will need to call into the surgery and speak with the receptionist.

We have been registering patients living outside the practice boundary under the “Out of Area scheme”.
Please note that we are not obliged to do home visits outside the area as part of this scheme. Instructions are available at reception for how to organise home visits if these are needed.
This only applies to patients already registered with us. If you are a New Patient wanting to register with us, and you are out of area, we are not obliged to accept you as a patient
