If you’ve been off sick for more than 7 days

To request a sick note:

Please book an appointment to discuss your sicknote with a Doctor or Advanced Nurse Practitioner first
If you need your sick note extending, please use the online request system here, scroll down to admin request and choose fit note. Please do this on the day that your sick note runs out, as we are unable to forward date sick notes.. The request will then be processed by one of our GPs and sent to you via your preferred contact method.

If you’ve been off sick for 7 days or less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
